The Loudest Silence At Rounds

I’m rounding with the endocrinology fellows this May as the “consultant of the month.” We have from 50 to 70 patients in our census. If I meet the fellows for two hours twice a week, we still won’t be able to go through all of them! Usually we discuss difficult cases and mortalities. I try to cultivate an atmophere where anyone is free to speak up. This comes from knowing that though I may be the oldest in the room, I am not necessarily always the wisest, or always correct.

At our rounds, we discussed how to work with others on the team, be it the residents or the fellows from other services. If the patient needs intubation, or requires a test, but the surgical/medical resident isn’t carrying out your orders, or the fellow of the other service signed out, what is one to do? When no one speaks up on the patient’s behalf, that is the loudest silence!
